Referrals are not necessary to visit Soma Health and Rehab. If you would like to see one of our Physiotherapists, please call or book online

Step Two

Have a read through our appointment expectations page to prepare for your appointment.

Step Three

After your initial appointment your Physiotherapist will let you know what your next steps are and will provide you with a management plan.

What you should expect

A welcoming environment

At Soma Health and Rehab our main objective is to get you back to doing what you love. Our approach is warm, caring and considerate and our practice reflects these values

Personal Questions

It is your choice what and how much information you choose to disclose In order to obtain a clear picture of you injury and impact on activities of daily living or function. The more relevant information you provide gives your physiotherapist more inside and assists in the diagnosis and management of your injury. Our staff adheres to the privacy and confidentiality act, and also understands the trusting relationship that is required for such disclosure of your personal information.

We may need to discuss your details with your GP or other relevant healthcare providers involved in your care.

Physical Contact

It is likely that physical contact will be necessary during the course of examination, assessment and treatment. If you feel uncomfortable at any time please inform your physiotherapist. Again, you may withdraw your consent at any time and any physical contact will cease immediately. Please inform your physiotherapist if anything can be done to assist your comfort or if you have any concerns.

Children and Minors

For the treatment of a minor this form must be signed by a custodian. Presence of a parent or caregiver is requested for anyone under the age of 16 years receiving treatment.

Treatment options and risks

Physiotherapy treatments are very low risk. Foreseeable risks will be discussed with you prior to administering treatment. You are always allowed to speak up. You may withdraw your consent at any time or request further treatment options.

What we expect of you

Arrive on time

In order to maintain a consistent and excellent approach to healthcare treatment we need to start and finish at the scheduled time. Please arrive on time for your appointments. If you are new to the clinic, arrive 10 mins prior to fill in any relavent paperwork.

Bring paperwork and reports

If you have had scans or have a report from a referring practitioner, please bring them along with you to your appointment. If applicable, please also bring any supporting paperwork in regards to DVA, workers compensation, NDIS and EPC claims.

Pay at time of consultation

You are responsible for your account at the time of your appointment. HICAPS facilities are available allowing instant rebates for most private health funds. Workers compensation and DVA patient accounts will be sent directly to the appropriate body. Should the claim be rejected in any way, you will be responsible for payment of accounts for any and all physiotherapy services received. We are not able to accept cash payments.

Refer your friends and family

The greatest compliment we can receive is the referral of a friend or family member. If you are happy with the service you have received at Soma Health and Rehab, we ask that you kindly refer your friends and family so they too can get the care they need.

Late cancellation = donation

Missed appointments are an inconvenience to the clinic, and often result in poorer outcomes for patients. Should any patient cancel or not attend a scheduled appointment without providing 24 hours’ notice, a fee of $40 will be charged. This fee is donated to local charity Cystic Fibrosis ACT.

Be committed to your rehab

Rehab involves a commitment to change or adjust your routine. It often means finding time to do something for yourself or begin a new exercise program. It can sometimes challenge previously help beliefs about what is going on and what you truely need to recover. You shouldn’t expect the work to be done for you. We will show you what to do.. the rest is on you.

Dress appropriately

Comfortable clothing with easy access to the affected area of the body is preferable. For example if you have an injury to your shoulder, please wear or pack with you a singlet to change into to allow for appropriate treatment.

Provide feedback
